is the current provider and this PPO Plan is offered to full-time employees only.
HYUNDAI POWER TRANSFOMERS USA pays 80% of the monthly family and single premium.
Salary Employees are eligible for coverage the first of the month following their date of hire.
Hourly Employees are eligible for coverage the first of the month following 90 days of their hire date.
Vision is included with the Health option, and cannot be separated.
· PRESCRIPTION COVERAGE:
When you use a participating pharmacy, prescription drugs will be covered at 100% subject to the following co-pays:
 
  · Generic Drugs: $10 co-pay per prescription 
  · Preferred Brand Name Drugs: $35 co-pay per prescription 
  · Other Brand Name Drugs: $50 co-pay per prescription.
· VISION COVERAGE:
There is a $200.00 maximum per person, per calendar year. 
· DENTAL: BLUE CROSS/BLUE SHIELD of ALABAMA

The dental plan is a freestanding plan, which only allows a one-time enrollment. There is no annual enrollment for dental.
· 401(k) RETIREMENT PLAN:
HYUNDAI POWER TRANSFOMERS USA 401(k) Retirement Plan is available to full-time and part-time employees who work a scheduled 30 hours per week. Employees are eligible to salary defer immediately.
An employee is eligible to receive the company's match of 2.5% up to 5% of the employees contributed earnings immediately. 401K vesting is monthly for members meeting current plan requirememnts enrolled in the plan by before tax contribution by method of HPT payroll deduction. After successful completion of 90 days employment.

A full-time employee earns vacation hours after the completion of a 90 day probationary period, based on years of service as follows:
